I keep seeing this all over the place. What the heck is it/does it mean?

Could be gratuitous icon post. THat is a twitter icon, and they have a limit of 140 characters per post, I believe. Sometimes done from a phone.

The o_O anime emoticon?

Sudden confusion, perhaps the one-eye-widening reaction when people read a Twitter message that's just come through as a SMS, maybe Twitter thought it looked like a birdie :-) Their previous default icon was a pale blue man with briefcase and umbrella, this I think is an improvement despite the fugly colours.

I like that there's a whole other set of emoticons that are eye-based rather than mouth-based ^_^

People keep inventing new emoticons too. For example o/\o is two people high fiving each other, which of course caused its comedy cousin o/ to come into use for one person putting up a hand while resigning themselves to nobody returning the gesture.
